Preventive Medicine

                       

The New York State Department of Health funding for the Long Island Occupational and Environmental

Health Center (LIOEHC) ended on March 31, 2013.  The LIOEHC was a program run by members of the

 Department of Preventive Medicine of Stony Brook School of Medicine which provides occupational,

 environmental and preventive medicine services to individual patients and employees of our corporate

 clients.  Our preventive medicine services include adult vaccinations and travel medicine services.

  Adult vaccinations include TDAP (tetanus with Pertussis) and the Shingles vaccine (Zostavax).

 

If you are a patient or company and would like to continue receiving services from the physicians of

Stony Brook Preventive Medicine Services, UFPC at Stony Brook Medicine, you may call our office at

 (631) 444-6250 to schedule an appointment.  If you were a LIOEHC patient, your medical records and

 history will be made available during the follow-up visit with a physician at Stony Brook Preventive

 Medicine Services.

 

In the event you plan to transfer your care to a physician in another medical practice, you will need to

 complete and sign a medical release form.  Click on the link below to print the release form, complete it
